Design

When the Apple Watch was released in April 2015, it offered only one set of internal features — call it "Series 0". Performance - Features

All versions of Apple Watch features a heart rate monitor, accelerometer, gyroscope and ambient light sensor. The Apple Watch 3 was  the first that supported an Altimeter while the Apple Watch 4 has ECG features for the first time. Comparing to Apple Watch 3, the new ECG feature is mostly likely the major upgrade of Apple Watch 4.

Each watch has featured 512MB of RAM, apart from Apple Watch 3 which features 768MB. The official details of Apple Watch 4 have not yet been released by Apple. I guess the most obvious difference must be the screen size and display.

Performance - Screen Display

The Apple Watch Series 4 has a much larger display with rounded edges, making full use of the space available and resulting in a bolder design. The 40mm Series 4 offers a 759sqmm display area compared to the 563sqmm on the 38mm Series 3.

The Apple Watch Series 4 offers a much bigger display that makes better use of the case size, an improved accelerometer, fall detection, Bluetooth 5.0 and an electrical heart sensor, which will eventually allow users to measure their own ECG (US/UK/Europe-only for now).

Performance - Speed

The big spec upgrade on the Apple Watch 2 is the S2 dual-core processor inside. For Apple Watch 3, we were still subjected to a spinning icon when trying to enter some apps although Apple said they already upgraded the processor.

The Apple Watch Series 4 has an upgraded processor that is claimed to be two times faster than the S3, called the S4. It also has the next generation wireless chip called the W3. Many of the hardware features are the same as Series 3, including the barometric altimeter, built-in GPS and the optical heart rate sensor, but the Series 4 does add a few extras.
